These forms are for the 2026-27 school year only!

To qualify for kindergarten you must turn 5 years old on or before Sept. 1, 2026.

Required Documents for All Grades (please have digital copies of these documents ready to upload):

• Birth Certificate
• Immunization Form
• Proof of Residence (water or electric bill matching the address/name of the person enrolling the child OR an affidavit from the Board of Education)
• Driver License (of parent/guardian)
• If Applicable:  Custody Paperwork
• For New Students Coming from Other Districts:  Copy of withdrawal paperwork from previous school

For Students Registering/Enrolling in Grades K-4, Please Verify School:

Your home address will determine which of Pell City’s four elementary schools your child will attend.  Click the “attendance zone verification” link below and enter your home address in the “find address/place” bar provided.  The map will indicate the name of the elementary school in your attendance zone (you may need to click off the pop-up box to see the school name).  This is the elementary school for which you will register your child.  If you need assistance with which school you are zoned, you may call 205-884-4800.

Additional Information Needed for Online Registration/Enrollment:

• Health insurance information
• Pediatrician and dentist information
• Emergency contact information (you may have up to five emergency contacts, which includes the parent)

During the online enrollment/registration process you will be asked to upload digital copies of many of the documents above.  This can be a PDF scanned copy or a picture taken with a mobile device/tablet.  

IMPORTANT NOTE:  A student is NOT officially registered until all requested documents have been received and/or uploaded and approved.
